Licensed Practical Nurse (LPN) - Home Care

Responsibilities

• Follow the plan of care (POC) developed by the Registered Nurse in partnership with the patient and caregivers
• Document accurate and ongoing treatments, including nursing interventions, patient responses, and progress toward goals
• Engage patients and caregivers in meaningful education as indicated by the care plan
• Communicate patient progress or changes in status to the interdisciplinary team and ordering providers
• Participate in agency projects, committees, and in-service programs
• Safely travel throughout the Granite VNA service area to provide home-based care

Requirements

• Graduate of an accredited school of Nursing
• Current New Hampshire nursing license or a compact state license through the Nurse Licensure Compact (NLC)
• One year of recent general medical/surgical or skilled/acute care nursing experience
• Valid driver's license, reliable transportation, and an acceptable driving record
