Licensed Practical Nurse (LPN)

Responsibilities

• Conduct and accurately document patient health histories and assessments
• Assist medical providers in delivering high-quality, patient-centered care
• Manage patient flow, including triage, follow-up care, and care coordination
• Coordinate referrals to specialists and community-based services
• Provide health education to patients and caregivers
• Collect and process laboratory specimens
• Support infection control practices and safety standards
• Assist with front office functions such as scheduling and patient discharge
• Float between Community Health Net locations as required

Requirements

• Active Pennsylvania Licensed Practical Nurse (LPN) license
• Current CPR certification
• High school diploma or GED
• Proficiency with computers and electronic health record (EHR) systems
• Knowledge of medical terminology and basic insurance types
