About the role
Responsibilities
- Administer medications and support daily care routines within LPN scope
- Support patients during admissions, discharges, activities of daily living (ADLs), and recovery-focused care activities
- Observe changes in patient behavior and report updates to the RN or physician
- Educate patients and families about care needs
- Collaborate with an interdisciplinary care team to support patient recovery, safety, and overall well-being
- Perform other nursing tasks as assigned by an RN
Requirements
- Current, active Kentucky LPN license
- High school diploma or GED
- Ability to meet all facility onboarding and clearance requirements
