Licensed Practical Nurse

Responsibilities

• Follow individual service plans and administer treatments prescribed by physicians
• Make meaningful rounds to residents, record visits, and document acute episodes or changes in status
• Administer injections, perform in-house testing, and take/record vital signs
• Prepare and administer prescribed medications; order new medications and refills
• Chart during shifts per facility policy, state guidelines, and resident status
• Supervise prescribed diets, fluid intake, and residents who self-administer medication
• Prepare incident reports and complete admission assessments and discharge documentation
• Assist residents with activities of daily living such as bathing, grooming, dressing, and escort service

Requirements

• Active LPN license
• Ability to lift at least 51 lbs regularly and provide physical assistance to residents
• Fluency in English (reading, writing, speaking, and understanding)
• Experience with memory care may be required if assigned to memory care areas
