About the role
Responsibilities
- Follow individual service plans and administer treatments prescribed by physicians
- Make meaningful rounds to residents, record visits, and document acute episodes or changes in status
- Administer injections, perform in-house testing, and take/record vital signs
- Prepare and administer prescribed medications; order new medications and refills
- Chart during shifts per facility policy, state guidelines, and resident status
- Supervise prescribed diets, fluid intake, and residents who self-administer medication
- Prepare incident reports and complete admission assessments and discharge documentation
- Assist residents with activities of daily living such as bathing, grooming, dressing, and escort service
Requirements
- Active LPN license
- Ability to lift at least 51 lbs regularly and provide physical assistance to residents
- Fluency in English (reading, writing, speaking, and understanding)
- Experience with memory care may be required if assigned to memory care areas
