Licensed Practical Nurse

Responsibilities

• Provide direct patient care for assigned patients under the direction of a Registered Nurse
• Administer intravenous medication in accordance with physician orders and state laws
• Assist in the identification, evaluation, and training of candidates for the home dialysis program
• Perform ongoing monitoring, assessment, and intervention for home patients, including home visits and clinical appointments
• Maintain accurate documentation of patient training and treatment in the Medical Information System
• Monitor patient supply procedures and ensure the maintenance of durable medical equipment
• Troubleshoot equipment problems with patients via telephone communication
• Collaborate with physicians and RN Coordinators to implement and adjust patient treatment plans

Requirements

• Graduate of an accredited School of Practical Nursing (LPN/LVN)
• Current and appropriate state nursing licensure
• Valid CPR certification
• Strong verbal and written communication skills

Preferred Qualifications

• Experience in medical-surgical nursing
• Experience in hemodialysis
• Experience in ICU settings
