About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Clinicians provide a continuum of care for those with chronic conditions and limited mobility through programs including remote patient monitoring and chronic care management.

Responsibilities

• Assist the Registered Nurse in carrying out the plan of care
• Assist in performing specialized procedures as directed by the Registered Nurse under physician's orders
• Assist with the preparation, implementation, and continuing evaluation of the patient plan of care
• Actively participate with the IDG to ensure the patient’s plan of care is kept current
• Reinforce teaching to the patient and family members regarding end of life care
• Foster cooperative effort among personnel through active participation in team and staff conferences

Requirements

• Graduate of an accredited School of Practical Nursing
• Licensed in VA as a Licensed Practical/Vocational Nurse by the Virginia State Board of Nursing
• One or more years of nursing experience
• Proven experience as a hospice LPN
• Licensed driver with an insured automobile in good working order
• Strong verbal and written communication skills
• Commitment to safety precautions and high ethical standards

Preferred Qualifications

• Hospice experience
• Bilingual skills
