About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Clinicians provide a continuum of care in the homes of patients with chronic conditions and limited mobility through programs like remote patient monitoring and chronic care management.

Responsibilities

• Assist the Registered Nurse in carrying out the plan of care
• Perform specialized procedures as directed by the Registered Nurse under physician's orders
• Assist with the preparation, implementation, and evaluation of patient care plans
• Collaborate with the Interdisciplinary Group (IDG) to ensure care plans are current
• Reinforce end-of-life care teaching to patients and family members
• Foster cooperative effort among personnel through active participation in team conferences

Requirements

• Graduate from an accredited School of Practical Nursing
• Licensed as a Licensed Practical/Vocational Nurse
• One or more years of nursing experience
• Proven experience as a hospice LPN
• Licensed driver with an insured automobile in good working order
• Strong verbal and written communication skills
• Commitment to safety precautions and high ethical standards
