Licensed Practical Nurse

Responsibilities

• Proactively manage patient health by monitoring remote patient monitoring (RPM) dashboards and responding to vital sign alerts
• Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage
• Coordinate timely follow-up care with Nurse Practitioners (NPs), Primary Care Providers (PCPs), or patient caregivers
• Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems
• Ensure patient compliance and troubleshoot remote monitoring device issues
• Act as the primary clinical point of contact for an assigned panel of remote care patients
• Assist with patient enrollment and onboarding activities
• Collaborate daily with the clinical team, intake staff, and support personnel

Requirements

• Active LPN license in Virginia
• Strong clinical judgment and excellent verbal communication skills
• Proficient with technology and managing multiple digital platforms
• Experience in Remote Patient Monitoring (RPM), telehealth, or complex care coordination is highly desirable
• Minimum internet speed of 15 Mbps with a 10 Mbps backup connection
• Desktop or laptop with Intel Core i5 (8th gen+) or AMD Ryzen 5 and minimum 8 GB RAM
