Licensed Practical Nurse

Responsibilities

• Attend Resident Care conferences and maintain accurate Resident Care Plans
• Recognize and identify signs and symptoms to coordinate interventions
• Communicate resident status to nursing staff, physicians, pharmacists, and families
• Prepare, administer, and document medications according to physician orders
• Ensure accurate documentation via electronic medical record platforms
• Provide general oversight and coordination of services during the shift
• Protect resident privacy and confidentiality

Requirements

• Colorado State Licensed Practical Nurse license in good standing
• Ability to work with different teams across various communities
• Proficiency in English (reading, writing, and speaking)
• Computer literacy including word processing and database experience
• Ability to handle objects up to 50 pounds occasionally

Preferred Qualifications

• 1 year of experience in a long-term care facility
• Experience coordinating care in a clinical environment
• Experience caring for residents with dementia
• IV Certification
• 1 year of experience using electronic medical record platforms
