Licensed Practical Nurse

Responsibilities

• Provide high-quality clinical services and technical skilled care within the patient's home in accordance with the plan of care
• Document patient visits thoroughly and completely in the electronic medical record per regulatory and payer requirements
• Actively participate in the care coordination process, including reporting changes in patient condition to the healthcare team
• Communicate effectively with patients, families, physicians, and agency management staff
• Follow all applicable infection control procedures and state nursing laws

Requirements

• Current and unrestricted LPN licensure in the state of practice
• Current driver's license and vehicle insurance
• Access to a dependable vehicle or ability to use public transportation

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• Previous experience in home care settings
• Ability to manage multiple tasks simultaneously and work independently
• Strong communication, writing, and organizational skills
• Ability to work flexible hours to meet client needs
