AVI Health & Community Services is a multi-disciplinary community-based organization providing HIV, hepatitis C, and harm reduction services across Vancouver Island. We work within a social justice and health promotion framework to ensure equity and access for all.

Responsibilities

• Provide care and support for individuals with high levels of poly-substance use and complex acute/chronic conditions
• Perform clinical assessments, screenings, and education regarding substance use, mental health, and sexual health
• Develop and maintain individualized care plans in collaboration with participants
• Provide harm reduction support, including risk reduction strategies and naloxone training
• Administer medications as ordered by program physicians according to clinical protocols
• Collaborate with community partners to ensure seamless support for participants
• Document care using Electronic Medical Records (EMR)

Requirements

• Licensed Practical Nurse (LPN) designation
• 1 to 5 years of nursing experience with poly-substance use and mental health challenges
• Experience in harm reduction, supervised consumption, or safer supply programs
• Demonstrated commitment to social justice, anti-racism, and decolonization
• Knowledge of medical approaches to substance use disorder and opiate agonist treatment
• Ability to work independently and as part of a multidisciplinary team
• Excellent communication and crisis management skills

Preferred Qualifications

• Valid Class 5 BC Driver’s License and access to a vehicle
• Lived expertise relevant to the position
