Responsibilities

• Provide comprehensive care in the patient's home, including skilled documentation
• Treat wounds and administer medications and treatments as prescribed
• Provide direct patient care including infusion therapy, teaching, and disease management
• Serve as patient advocate and maintain communication with physicians, caregivers, and facility management
• Participate in regular case conferences and in-services
• Manage a caseload of approximately 6 patients per day, Monday through Friday

Requirements

• Current Texas LPN license
• Three years of clinical nursing experience
• Current Texas driver's license
• CPR/BLS Certification
• Experience with healthcare software
• Ability to perform skilled procedures within the scope of the Nurse Practice Act
• Strong organization, time management, and multitasking skills

Preferred Qualifications

• Previous home health experience
