About the role
Responsibilities
- Evaluate adult patients' mobility, strength, and functionality in a home-based setting
- Design and execute custom rehabilitation plans
- Guide patients and families on home exercise routines, body mechanics, and injury prevention
- Maintain accurate clinical documentation
- Coordinate with an interdisciplinary healthcare team to optimize outcomes
Requirements
- Degree in Physical Therapy from an accredited program
- Valid Arizona PT license or license-eligible
- Strong clinical assessment skills
- Excellent communication skills
- Ability to lift/assist patients and perform physical tasks inherent to mobility therapy
