Licensed Nursing Assistant

Responsibilities

• Perform delegated nursing, homemaking, and other duties for patients of all ages in their homes or designated facilities
• Report and document patient needs and responses to the appropriate Case Manager or Clinical Manager
• Immediately report any changes in patient condition or critical situations
• Adhere to established Home Health Aide care plans while maintaining a safe and healthy environment
• Reinforce teaching from primary nurses or therapists by reviewing personal care techniques with caregivers and patients
• Complete all required documentation in clinical records, time sheets, and quality control logs
• Provide care that promotes patient dignity and maintains a calm, supportive, and therapeutic environment

Requirements

• Current Nursing Assistant License in the State of New Hampshire
• High school diploma or equivalent
• Valid Driver's License
• Current automobile insurance coverage

Preferred Qualifications

• Six months to one year of experience in a related healthcare field
