Licensed Nursing Assistant (LNA) - Home Health and Hospice

Responsibilities

• Perform personal care and designated health-related services for Home Health and Hospice patients in their homes.
• Assist clients with Activities of Daily Living (ADL) including walking, bathing, grooming, eating, and incontinence management.
• Observe, record, and report changes in resident condition or status to the nursing staff in a timely manner.
• Document care provided accurately in accordance with agency guidelines.
• Assist with homemaking tasks, medication assistance, and wound care as outlined in the care plan.
• Communicate professionally and courteously with clients, family members, and interdisciplinary healthcare team members.

Requirements

• High school diploma or GED.
• Current New Hampshire LNA license.
• One year of recent experience as a Licensed Nursing Assistant.
