Licensed Nursing Assistant (LNA) - Home Care and Hospice

Responsibilities

• Perform personal care and designated health-related services for Home Health and Hospice patients in their homes or facilities
• Assist clients with Activities of Daily Living (ADL) including walking, bathing, grooming, eating, and incontinence management
• Observe, record, and report changes in resident condition or status to the nursing staff in a timely manner
• Document all care provided accurately in accordance with agency guidelines
• Assist with homemaking tasks, medication assistance, and wound care as outlined in the care plan
• Communicate professionally and courteously with clients, family members, and interdisciplinary healthcare team members

Requirements

• High school diploma or GED
• Current New Hampshire Licensed Nursing Assistant (LNA) license
• One year of recent experience as a Licensed Nursing Assistant
