Licensed Nursing Assistant (LNA) - Facilities

Responsibilities

• Provide personal care and designated health-related services to clients under RN supervision
• Assist with Activities of Daily Living (ADL) including walking, bathing, grooming, eating, and incontinence management
• Observe, record, and report changes in resident condition or status to nursing staff
• Document care provided accurately in accordance with agency guidelines
• Perform homemaking tasks, medication assistance, and wound care as outlined in the care plan
• Maintain professional and supportive communication with clients, families, and healthcare team members

Requirements

• High school diploma or GED
• Current New Hampshire LNA license
• One year of recent experience as a Licensed Nursing Assistant
