Licensed Master Social Worker

Responsibilities

• Provide psychosocial services and supportive counseling to dialysis patients and their families
• Conduct patient assessments to identify psychosocial needs, strengths, and barriers to treatment
• Collaborate with an interdisciplinary healthcare team to develop and implement care plans
• Educate patients and families on kidney disease, treatment modalities, and transplant processes
• Facilitate referrals to community resources, including vocational rehabilitation and home health services
• Assist patients with advance directives and participate in discussions regarding DNR status
• Monitor patient adherence and quality of life indicators to support optimal treatment outcomes
• Coordinate with insurance professionals to address patient coverage and financial assistance needs

Requirements

• Master's degree in Social Work (MSW)
• Valid state-required social work license
• Knowledge of Social Work Theory of Human Behavior
• 0–2 years of related experience (entry-level)
