Licensed Master Social Worker

Responsibilities

• Provide psychosocial services and supportive counseling to dialysis patients and their families to improve quality of life.
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to treatment.
• Collaborate with the interdisciplinary healthcare team to develop and implement effective care plans.
• Educate patients and families on kidney disease, treatment modalities, and available support services.
• Facilitate referrals to community resources, including vocational rehabilitation and home health services.
• Assist patients with the transplant referral process and manage transplant waitlist coordination.
• Support patient understanding of advance directives and participate in DNR status discussions with physicians.
• Monitor and report on quality indicators such as treatment adherence and patient satisfaction.
• Coordinate with insurance specialists to address patient coverage, billing, and financial assistance needs.

Requirements

• Master's degree in Social Work (MSW).
• Valid state-required social work license.
• Knowledge of social work theory and human behavior.
• 0–2 years of related experience (entry-level).
