Lead Physiotherapist

Responsibilities

• Lead the development and delivery of the Discharge to Assess (D2A) model across acute and community settings
• Provide advanced clinical leadership and autonomous decision-making for complex discharge planning
• Collaborate with health and social care professionals to ensure seamless, patient-centred transitions
• Deliver specialist physiotherapy assessments and interventions across inpatient and community environments
• Support governance, quality assurance, and service innovation within the Integrated Discharge Team (IDT)
• Contribute to strategic planning, workforce development, and training across the CTH
• Participate in 7-day service delivery and rotational work with inpatient and community therapy teams

Requirements

• HCPC registration
• Degree in Physiotherapy
• Extensive post-registration experience in acute, community, or social care settings
• Proven leadership in complex case management and discharge planning
• Strong understanding of D2A pathways, NICE guidelines, and intermediate care models
• Experience in MDT working, staff supervision, and service development
• Previous NHS experience essential
