Intensive Case Manager

Responsibilities

• Provide community outreach to each individual as part of the multi-disciplinary team
• Offer side-by-side interventions focusing on problem-solving, skill training, modeling, optimism, and encouragement
• Collaborate with the team and each individual to develop a person-centered treatment plan
• Support individuals in becoming fully integrated into their communities by helping them build natural supports
• Empower individuals to take control of their lives, focusing on treatment, leisure, employment, and personal recovery
• Provide on-call coverage as part of a rotation during evenings and weekends

Requirements

• Bachelor's or Master's degree in Psychology, Social Work, or a related field
• Experience working with adults with mental illness
• Valid Massachusetts driver's license
