Inpatient Social Worker

Responsibilities

• Provide assessment and evaluation of patients regarding clinical, psychological, and social factors contributing to their disease process.
• Interface with physicians, nurses, community-based facilities, and financial institutions to manage cases and secure necessary resources.
• Support patients and families by coordinating care planning, patient safety, and transitional care needs.
• Consult with Inpatient Registered Nurse Care Managers regarding medically complex discharge plans.
• Manage the care of simple to complex multi-system patients using efficient time and resource management.
• Document measurement and evaluation of financial, clinical, functional, and satisfaction outcomes.
• Provide leadership to the healthcare team regarding discharge and care planning.

Requirements

• Master's Degree in Social Work or a related field.
• 10 years of experience in an acute care facility.
• Proven experience in care management and discharge planning.
• Strong interpersonal, organizational, and motivational skills.
• Proficiency in written and verbal communication.
• Ability to function effectively as a team player.
