Inpatient Registered Nurse Care Manager

Responsibilities

• Assess and evaluate patients for clinical, psychological, and social factors contributing to their disease process and care needs.
• Perform initial and concurrent reviews to determine appropriateness of status, severity of illness, and intensity of treatment.
• Collaborate with a multidisciplinary team, including physicians, social workers, and community-based facilities, to manage cases and secure necessary resources.
• Coordinate care planning and discharge planning to ensure safe transitions of care for patients and families.
• Interface with third-party payers to provide required documentation of medical necessity and manage utilization reviews.
• Document clinical, functional, and satisfaction outcomes in the Electronic Medical Record (EMR).
• Provide leadership to the healthcare team regarding discharge planning and patient safety.

Requirements

• Associate degree in Nursing from an accredited program.
• Current New Hampshire Registered Nurse (RN) license.
• Minimum of 2 years of experience in an acute care facility.
• Experience in Care Management, Discharge Planning, Quality Improvement, or Utilization Review.
• Strong interpersonal, organizational, and motivational skills.
• Proficient written and verbal communication skills.

Preferred Qualifications

• Bachelor's degree in Nursing (BSN).
