About the role
Responsibilities
- Provide direct 1:1 support to Indigenous patients through coordination of health and social services
- Develop comprehensive care plans addressing medical and social needs, including navigation and advocacy
- Facilitate access to Indigenous ceremonies, medicines, Elders, and Traditional Healers
- Build and sustain relationships with community partners and organizations
- Support data collection and tracking to improve understanding of client experiences
- Contribute to the development of screening, intake, and referral processes
- Serve as a resource for staff regarding the needs of medically and socially complex patients
- Support access to education on health equity and trauma-informed care
Requirements
- Bachelor's degree in a relevant health or social work field or equivalent lived/work experience
- Minimum 4 years of practical and related experience
- Strong knowledge of community resources for Indigenous peoples and social determinants of health
- Experience building relationships with Indigenous leaders, Elders, and Knowledge holders
- Experience with trauma-informed care and anti-racist/anti-discriminatory practices
- Excellent interpersonal, oral, and written communication skills
- Ability to work both independently and as part of an interdisciplinary team
Preferred Qualifications
- Fluency in an Indigenous language (written or verbal)
- Experience in program development, implementation, and evaluation
- Understanding of acute care settings and hospital systems
- Facilitation or teaching experience
