About the role

Responsibilities

  • Provide direct 1:1 support to Indigenous patients through coordination of health and social services
  • Develop comprehensive care plans addressing medical and social needs, including navigation and advocacy
  • Facilitate access to Indigenous ceremonies, medicines, Elders, and Traditional Healers
  • Build and sustain relationships with community partners and organizations
  • Support data collection and tracking to improve understanding of client experiences
  • Contribute to the development of screening, intake, and referral processes
  • Serve as a resource for staff regarding the needs of medically and socially complex patients
  • Support access to education on health equity and trauma-informed care

Requirements

  • Bachelor's degree in a relevant health or social work field or equivalent lived/work experience
  • Minimum 4 years of practical and related experience
  • Strong knowledge of community resources for Indigenous peoples and social determinants of health
  • Experience building relationships with Indigenous leaders, Elders, and Knowledge holders
  • Experience with trauma-informed care and anti-racist/anti-discriminatory practices
  • Excellent interpersonal, oral, and written communication skills
  • Ability to work both independently and as part of an interdisciplinary team

Preferred Qualifications

  • Fluency in an Indigenous language (written or verbal)
  • Experience in program development, implementation, and evaluation
  • Understanding of acute care settings and hospital systems
  • Facilitation or teaching experience
