About the role

Responsibilities

  • Lead the end-to-end implementation of the Neighbourhood Health Hub (NHH) model across all five stages
  • Apply implementation science frameworks (such as CFIR, ERIC, and Proctor outcomes) to guide service delivery
  • Coordinate the staged rollout of Small Teams, Team of Teams, and Hub Facility models
  • Build and maintain strong cross-sector partnerships with health, NGO, council, and community stakeholders
  • Embed co-design principles by engaging people with lived experience and priority populations
  • Use local data, asset mapping, and evaluation frameworks to drive informed decision-making
  • Support commissioning alignment, sustainability planning, and capability building across providers
  • Contribute to the development of tools, frameworks, and reporting to support system-wide implementation

Requirements

  • Tertiary qualification in health, public health, community services, psychology, social work, implementation science, project management, or a related field
  • Proven experience in implementation, service development, or project coordination within the health or community services sector
  • Experience managing multi-site or community-based implementation projects
  • Demonstrated experience in co-design, stakeholder consultation, and developing practical resources or training materials
  • Strong ability to gather and synthesise qualitative data from stakeholder engagement and service delivery activities
  • Excellent written communication skills, specifically in preparing professional reports and summaries
  • Strong organisational and project management skills with the ability to manage competing priorities
