Hub Home Provider

Responsibilities

• Organise, coordinate, and host monthly Constellation meetings at the Hub Home, including meals and community business.
• Coordinate and host at least one quarterly social outing for satellite carers and children/young people.
• Host meetings at the home, such as case planning, care team meetings, and family visits.
• Maintain regular contact with satellite families to assist with placement issues, problem-solving, and accessing community resources.
• Provide support to satellite families in developing success plans for children, including mental health and school resources.
• Maintain accurate records of services provided, engagement levels, and recruitment activities.

Requirements

• Successful authorisation/registration to act as a foster carer under the relevant jurisdiction.
• Significant experience as a foster/kinship carer (5+ years preferred) or professional experience in the Child Protection or Out of Home Care sector.
• A home environment that is conducive and appropriate to support satellite families.
• Current First Aid Certificate.
• Current Driver's licence.
• Demonstrated emotional maturity and capacity for relationship building with carers and children.
