Housing Specialist

Responsibilities

• Support members through all stages of the housing process, from assessment through placement
• Assist members facing rental arrears or risk of eviction by coordinating housing stabilization and eviction prevention resources
• Prepare, submit, and track housing applications and required documentation
• Coordinate housing interviews and prepare members for the interview process
• Communicate with housing providers, landlords, and community partners
• Track housing goals and progress within members’ Plans of Care
• Build and maintain relationships with housing providers and community-based organizations
• Document all housing-related services accurately in designated systems and EHRs
• Participate in care planning meetings and interdisciplinary case conferences

Requirements

• At least 1 year of experience in housing placement, housing navigation, or eviction prevention
• Experience navigating the legal housing system in NY
• High School Diploma or equivalent
• Proficiency with Microsoft Office and care management systems
• Ability to work both in-office and in the field

Preferred Qualifications

• Experience with rental assistance, HRA, or housing stabilization programs
• Familiarity with NYC housing systems or subsidies
• Associate’s or Bachelor's degree
• Bilingual abilities
