Requirements

About the role

Responsibilities

  • Facilitate the transition of individuals from the street to permanent housing using a Housing First approach
  • Conduct client intake, assessments, and preliminary goal setting to identify barriers to housing
  • Provide crisis intervention and coaching to support clients in accessing appropriate services
  • Perform active housing searches and attend viewings with clients to procure stable housing
  • Maintain accurate records, including incident reports, log notes, and case notes
  • Advocate for clients with community resources, landlords, and service providers
  • Monitor client progress and maintain ongoing contact to ensure goal achievement
  • Manage program resources such as client spending, mileage, and necessary supplies

Requirements

  • Community College degree in Social Work or equivalent work experience
  • Minimum 2 years of direct case management experience working with urban homeless populations
  • Experience working with individuals experiencing addiction and/or mental health issues
  • Strong motivational interviewing, written communication, and computer skills
  • Valid G driver's license and a current clean driver's abstract
  • Ability to handle high-stress situations and competing priorities
  • Ability to pass a clear Police Reference check for the Vulnerable Sector

Preferred Qualifications

  • Current certification in First Aid/CPR, ASSIST, and NVCI
  • Experience administering Narcan or providing Narcan training
  • Bilingualism in English and French
