HouseCalls Nurse Practitioner or Physician Asst.

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, and physical exams
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses to assist with care management and treatment planning
• Communicate clinical findings to members' primary care providers to address gaps in care
• Educate members on disease processes, medications, and treatment compliance
• Address social determinants of health and provide necessary referrals
• Recognize and intervene in urgent or emergent situations appropriately

Requirements

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state of practice without disciplinary actions
• Active and unrestricted driver's license
• Access to reliable transportation for conducting home visits
• Ability to navigate varied home environments, including sitting, standing, or kneeling as needed

Preferred Qualifications

• 1+ years of clinical experience, specifically in family, geriatric, or home health settings
• Proficiency with electronic medical records (EMR) and healthcare technology
• Ability to transport equipment weighing up to 30 pounds and navigate stairs
• Proven communication skills with geriatric or Medicare populations
