Hospital Social Worker

Responsibilities

• Support residents admitted to hospitals within and outside the borough
• Deliver strength-based preventative support and assess needs
• Establish support systems and safeguard patients
• Review care packages and consider placements
• Refer individuals to reablement services to ensure timely discharge
• Collaborate with multidisciplinary colleagues, voluntary sector organisations, and wider agencies

Requirements

• Social Work England registration
• Degree-level qualification or equivalent in Social Work
• At least 2 years of post-qualified experience
• Full UK driving license
• Eligibility to work in the UK
• Experience with adults with physical health disabilities and vulnerabilities
• Robust understanding of safeguarding procedures
