Hospital in the Home Coordinator

Responsibilities

• Coordinate patient transitions into HITH and CITH services
• Provide clinical leadership, education and advice to nursing and multidisciplinary teams
• Build strong relationships with internal and external referrers
• Support service development, quality improvement and integrated models of care
• Ensure patients receive safe, high-quality, person-centred care throughout their journey
• Contribute to revenue optimisation through effective management of health fund processes and service access pathways

Requirements

• Current AHPRA registration as a Registered Nurse
• Previous Hospital in the Home (HITH) nursing experience
• Demonstrated clinical expertise in coordinating complex patient care
• Knowledge of the private health environment and health fund arrangements
• Strong leadership, communication and stakeholder engagement skills
• Current Victorian driver's licence

Preferred Qualifications

• Postgraduate qualifications
• Management experience
