Hospice Weekend RN

Responsibilities

• Triage patient needs over the phone until an in-person visit can be provided
• Respond with urgency to afterhours crisis calls from patients and families
• Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral
• Perform afterhours visits including admissions, death visits, recertifications, and routine visits
• Identify and reassess patient and family physical, psychosocial, emotional, and environmental needs every 15 days
• Document patient problems, goals, interventions, and responses to care

Requirements

• Current unrestricted RN licensure in the state of practice
• Current CPR Certification or ability to obtain within 90 days of hire
• Current driver's license and valid vehicle insurance
• Access to a dependable vehicle or public transportation
• Ability to function in diverse home environments

Preferred Qualifications

• 1+ years of clinical experience
