Hospice Social Worker

Responsibilities

• Conduct initial and ongoing psychosocial, spiritual, and pre-bereavement assessments for patients and families.
• Develop and implement personalized care plans, including counseling for anticipatory grief and resource referrals.
• Provide case management services to address the psychosocial needs of patients in private residences, assisted living, and skilled nursing facilities.
• Identify and support high-risk or suicidal patients and families experiencing non-medical crises.
• Complete timely clinical documentation, including psychosocial assessments, clinical notes, and bereavement reports.
• Participate in interdisciplinary team meetings and case conferences to enhance patient care.
• Collaborate with the Volunteer Coordinator to match patient needs with appropriate volunteer assignments.

Requirements

• Master's Degree in Social Work (MSW) from an accredited institution.
• Experience in individual, family, and group treatment modalities.
• Valid driver's license and reliable transportation for field visits.
• Proof of automobile insurance.
• Ability to maintain professional ethics, confidentiality, and a calm demeanor during crisis situations.

Preferred Qualifications

• One year of post-MSW experience in a hospital, home health, or hospice setting.
• Minimum of three years of experience in the provision of community social services.
• Two years of experience in psychosocial assessment of patients and families.
• Theoretical knowledge of human development, end-of-life care, and family systems.
