Hospice Social Worker

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Their clinicians provide a continuum of care for patients with chronic conditions and limited mobility, offering programs such as remote patient monitoring and behavioral health management.

Responsibilities

• Conduct psychosocial assessments including emotional, social, financial, and environmental resources
• Participate in IDG meetings and the development of patient care plans
• Observe, record, and report changes in patient emotional and social factors
• Maintain and submit written clinical records, including initial evaluations and daily notes
• Evaluate patient and family response to medical social work interventions
• Coordinate visit scheduling with Case Managers
• Participate in discharge planning and QAPI meetings
• Assist the health team in understanding social and environmental factors related to patient health

Requirements

• Master's of Social Work (MSW)
• At least one year of post-bachelor’s social work experience in a healthcare setting
• Licensed driver with an insured automobile in good working order
• Strong verbal and written communication skills
• Ability to maintain confidential information

Preferred Qualifications

• Previous hospice experience
