Hospice Social Worker

Responsibilities

• Conduct psychosocial assessments within 5 days of hospice admission
• Participate in IDG meetings and patient plan of care development
• Monitor and report changes in patient emotional and social factors
• Maintain clinical records including evaluations, care plans, and daily notes
• Evaluate effectiveness of medical social work interventions
• Coordinate visit scheduling with Case Managers
• Assist patients in utilizing family and community resources
• Act as a consultant to the health team regarding social and environmental factors

Requirements

• Master's degree in Social Work (MSW) from a CSWE accredited program or a CSWE accredited Bachelor's degree meeting §418.114(b)(3) qualifications
• Minimum of one year of healthcare experience
• Licensed driver with a reliable, insured automobile
• Strong verbal and written communication skills

Preferred Qualifications

• Previous experience in hospice care
• Deep understanding of hospice philosophy and end-of-life issues
