Hospice Social Worker

Responsibilities

• Conduct psychosocial assessments within 5 days of hospice admission
• Participate in IDG meetings and patient plan of care development
• Observe and report changes in patient emotional and social factors
• Maintain and submit clinical records including evaluations and daily notes
• Evaluate patient and family response to medical social work interventions
• Coordinate visit scheduling with Case Managers
• Assist in discharge planning and support the IDG team
• Help patients utilize family and community resources

Requirements

• Master's Degree in Social Work (MSW) or Bachelor's Degree in Social Work (BSW)
• One year of Social Work experience in a healthcare setting
• Licensed driver with an insured automobile in good working order
• Strong verbal and written communication skills

Preferred Qualifications

• Hospice experience
