About the role
Responsibilities
- Conduct psychosocial assessments including emotional, social, financial, and environmental resources
- Participate in IDG meetings and patient plan of care development
- Observe and report changes in patient emotional and social factors
- Maintain and submit clinical records, including evaluations and daily notes
- Evaluate effectiveness of medical social work interventions
- Coordinate visit scheduling with Case Managers
- Assist in discharge planning and act as a consultant to the health team
- Help patients utilize family and community resources
Requirements
- Licensed by the NJ State Board of Social Work Examiners
- Master’s degree in Social Work from a CSWE-accredited school
- At least one year of post-master’s social work experience in a healthcare setting
- Licensed driver with an insured automobile in good working order
- Strong verbal and written communication skills

- Previous hospice experience
