Hospice Social Worker

Responsibilities

• Conduct psychosocial assessments including emotional, social, financial, and environmental resources
• Participate in IDG meetings and patient plan of care development
• Observe and report changes in patient emotional and social factors
• Maintain and submit clinical records, including evaluations and daily notes
• Evaluate effectiveness of medical social work interventions
• Coordinate visit scheduling with Case Managers
• Assist in discharge planning and act as a consultant to the health team
• Help patients utilize family and community resources

Requirements

• Licensed by the NJ State Board of Social Work Examiners
• Master’s degree in Social Work from a CSWE-accredited school
• At least one year of post-master’s social work experience in a healthcare setting
• Licensed driver with an insured automobile in good working order
• Strong verbal and written communication skills

Preferred Qualifications

• Previous hospice experience
