Hospice Social Worker

Responsibilities

• Conduct psychosocial assessments including emotional, social, financial, and environmental resources
• Participate in IDG meetings and patient plan of care development
• Observe, record, and report changes in patient emotional and social factors
• Maintain and submit clinical records, including initial evaluations and daily notes
• Evaluate patient and family response to medical social work interventions
• Coordinate visit scheduling with Case Managers
• Participate in discharge planning and QAPI meetings
• Assist health team members in understanding social and environmental factors related to patient health
• Help patients utilize family and community resources

Requirements

• Bachelor's in Social Work (BSW)
• Licensed driver with an insured automobile in good working order
• Strong verbal and written communication skills
• Ability to maintain confidential information

Preferred Qualifications

• Previous hospice experience
