Hospice Social Worker

Responsibilities

• Conduct psychosocial assessments including emotional, social, financial, and environmental resources
• Participate in IDG meetings and patient plan of care development
• Observe and report changes in patient emotional and social factors
• Maintain and submit clinical records, including evaluations and daily notes
• Evaluate effectiveness of medical social work interventions
• Coordinate visit scheduling with Case Managers
• Assist in discharge planning and support the IDG in patient care
• Act as a consultant to the health team regarding social and environmental factors
• Help patients utilize family and community resources

Requirements

• Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) in NJ
• Bachelor's degree in social work from a CSWE accredited school
• At least one year of post-master's social work experience in a healthcare setting
• Licensed driver with an insured automobile
• Strong verbal and written communication skills
• Ability to maintain confidential information

Preferred Qualifications

• Previous hospice experience
