About the role
Responsibilities
- Conduct psychosocial assessments including emotional, social, financial, and environmental resources
- Participate in IDG meetings and the development of patient care plans
- Observe and report changes in patient emotional and social factors
- Maintain and submit clinical records, including evaluations and daily notes
- Evaluate patient and family response to medical social work interventions
- Coordinate visit scheduling with Case Managers
- Assist in discharge planning and support the IDG in patient care
- Act as a consultant to the health team regarding social and environmental factors
Requirements
- Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) in NJ
- Bachelor's degree in social work from a CSWE accredited school
- At least one year of post-master's social work experience in a healthcare setting
- Licensed driver with an insured automobile
- Strong verbal and written communication skills

- Previous hospice experience
