Hospice RN - PRN

Responsibilities

• Assume full nursing responsibility for the delivery of the Plan of Care for hospice patients
• Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral
• Identify and re-assess physical, psychosocial, and environmental needs of patients and families at least every 15 days
• Coordinate and implement treatment strategies to promote physical, psychosocial, emotional, and spiritual well-being
• Collaborate with attending physicians and the Interdisciplinary Group (IDG) to provide daily patient care
• Document clinical notes, goals, interventions, and patient responses accurately
• Instruct and supervise patients and families in appropriate self-care techniques

Requirements

• Current and unrestricted RN licensure in the state of practice
• 1+ years of clinical experience
• Valid driver's license and access to a dependable vehicle or public transportation

Preferred Qualifications

• Current CPR Certification (or ability to complete within 90 days of hire)
• Demonstrated ability to work independently
• Strong communication, writing, and organizational skills
