About the role
Responsibilities
- Provide skilled nursing care and compassionate support to patients and families
- Coordinate comprehensive care for assigned caseload
- Build rapport with patients, families, physicians, and interdisciplinary team members
- Assess and respond to the needs of patients in homes, assisted living, or nursing facilities
- Manage multiple priorities, documentation requirements, and deadlines
Requirements
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse
- Minimum one year of experience as a nurse in a clinical care setting
- Reliable transportation, valid driver’s license, and valid automobile insurance
- Ability to travel daily within the service area
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Hospice or home health experience
