Hospice RN Case Manager

Responsibilities

• Provide skilled nursing care and compassionate support to patients and families
• Coordinate comprehensive care for a caseload
• Work collaboratively with an interdisciplinary team to address physical, emotional, and spiritual needs
• Build rapport with patients, families, physicians, and facilities
• Travel daily within the service area to provide care in homes, assisted living, or nursing homes

Requirements

• Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one year of experience as a nurse in a clinical care setting
• Reliable transportation, valid driver’s license, and valid automobile insurance
• Ability to handle multiple priorities, documentation, and deadlines

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Hospice or home health experience
