Hospice RN Case Manager

Responsibilities

• Manage assigned patient caseload including assessment, planning, implementation, and evaluation of care
• Coordinate with attending physicians, hospice staff, and other agencies to optimize patient care and resource use
• Maintain accurate and timely documentation in electronic medical records
• Participate in interdisciplinary group meetings and patient care conferences
• Provide direct care and teach primary caregivers, volunteers, and employed caregivers as appropriate
• Supervise and document the NAC every two weeks or as assigned
• Provide support at time of death and during bereavement periods
• Participate in hospice orientation, in-service training, and community programs

Requirements

• Graduate from an accredited School of Nursing
• Current NJ Registered Nurse license
• One year of RN experience; hospice experience preferred
• Excellent assessment, problem-solving, and time management skills
• Knowledge of case management processes
• Valid driver's license, insured automobile, and good driving record
• Strong verbal and written communication skills
