Hospice RN Case Manager

Responsibilities

• Manage a patient caseload including assessment, planning, implementation, and evaluation
• Coordinate care with attending physicians, hospice staff, and other agencies
• Provide accurate and timely documentation in electronic medical records
• Participate in Interdisciplinary Group meetings and patient care conferences
• Coordinate physical care by teaching caregivers or providing direct care
• Supervise and document NAC every two weeks
• Provide support during time of death and bereavement periods

Requirements

• Graduate from an accredited School of Nursing
• Currently licensed Registered Nurse in Georgia
• One year of RN experience
• CPR certification for patient care
• Licensed driver with an insured automobile
• Excellent assessment and problem-solving skills

Preferred Qualifications

• Previous hospice experience
• Knowledge of case management processes
