About the role
Responsibilities
- Manage a patient caseload including assessment, planning, implementation, and evaluation
- Coordinate care with attending physicians, hospice staff, and other agencies
- Provide accurate and timely documentation in electronic medical records
- Participate in Interdisciplinary Group meetings and patient care conferences
- Coordinate physical care by teaching caregivers or providing direct care
- Supervise and document NAC every two weeks
- Provide support during time of death and bereavement periods
Requirements
- Graduate from an accredited School of Nursing
- Currently licensed Registered Nurse in Georgia
- One year of RN experience
- CPR certification for patient care
- Licensed driver with an insured automobile
- Excellent assessment and problem-solving skills
Preferred Qualifications
- Previous hospice experience
- Knowledge of case management processes
