Hospice RN Case Manager

About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility.

Responsibilities

• Assume primary responsibility for a patient caseload including assessing, planning, implementing, and evaluating nursing processes
• Coordinate care by communicating with attending physicians, hospice staff, and other agencies
• Provide accurate and timely documentation in electronic medical records
• Coordinate physical care by teaching caregivers and volunteers or providing direct care
• Supervise and document the NAC every two weeks
• Provide support during time of death and bereavement periods
• Participate in Interdisciplinary Group meetings and patient care conferences

Requirements

• Graduate of an accredited school of nursing
• 1-2 years of recent acute care experience in an institutional setting
• 1-2 years of recent experience in home care
• Current licensure in Kansas
• Licensed driver with a reliable, insured automobile
• Ability to lift, position, and transfer patients and equipment

Preferred Qualifications

• Bachelor’s degree
• 1 year of hospice care experience
