Hospice RN Case Manager

About the Company

Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility through various programs including remote patient monitoring and chronic care management.

Responsibilities

• Assume primary responsibility for a patient caseload including assessing, planning, implementing, and evaluating nursing processes
• Initiate communication with attending physicians and hospice staff to coordinate optimal care
• Provide accurate and timely documentation in electronic medical records
• Coordinate physical care by teaching primary caregivers and volunteers
• Participate in Interdisciplinary Group meetings and patient care conferences
• Provide support at the time of death and during the bereavement period
• Maintain familiarity with agency policies and state/federal hospice regulations

Requirements

• Graduate from an accredited School of Nursing
• Currently licensed Registered Nurse in the state of Georgia
• One year of RN experience (Hospice experience preferred)
• CPR certification
• Licensed driver with an insured automobile in good working order
• Strong assessment, problem-solving, and time management skills
