About the role
About the Company
Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians provide a continuum of care for patients with chronic conditions and limited mobility directly in their homes.
Responsibilities
- Assume primary responsibility for a patient caseload including assessment, planning, implementation, and evaluation
- Coordinate care with attending physicians, hospice staff, and other agencies
- Provide accurate and timely documentation in electronic medical records
- Coordinate physical care by teaching caregivers or providing direct care
- Participate in Interdisciplinary Group meetings and patient care conferences
- Provide support during the time of death and bereavement periods
- Supervise and document NAC every two weeks
Requirements
- Graduate from an accredited School of Nursing
- Licensed Registered Nurse in New Jersey
- One year of RN experience (Hospice experience preferred)
- Fluent in both English and Spanish
- Licensed driver with an insured automobile in good working order
- Strong assessment, problem-solving, and time management skills
