About the role
Responsibilities
- Manage a patient caseload including assessing, planning, implementing, and evaluating nursing processes
- Coordinate care with attending physicians, hospice staff, and other agencies
- Provide accurate and timely documentation in electronic medical records
- Conduct Goals of Care discussions with patients
- Participate in discharge planning for GIP and RHC discharges into the community
- Perform hospice evaluations and provide daily GIP level of care visits
- Support routine home visits and admissions in the field as needed
- Provide support during time of death and bereavement periods
Requirements
- Current license to practice as a Registered Nurse in the State of Georgia
- 1-2 years of professional nursing experience, preferably in hospice care
- Valid driver's license and reliable vehicle for year-round travel
- Ability to work independently and as part of a multidisciplinary team
- Strong assessment and clinical documentation skills
