About the role
About the Company
Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Our clinicians go to the home of the patient, providing a continuum of care for those with chronic conditions and limited mobility.
Responsibilities
- Assume primary responsibility for a patient caseload including assessing, planning, implementing, and evaluating nursing processes
- Initiate communication with attending physicians and hospice staff to coordinate optimal care
- Provide accurate and timely documentation in electronic medical records
- Coordinate physical care by teaching primary caregivers and volunteers
- Participate in Interdisciplinary Group meetings and patient care conferences
- Provide support at the time of death and during the bereavement period
- Supervise and document NAC every two weeks
Requirements
- Graduate from an accredited School of Nursing
- Licensed in NJ as a Registered Nurse
- One year of RN experience (Hospice experience preferred)
- Licensed driver with an insured automobile in good working order
- Fluent in both English and Spanish
- Excellent assessment and problem-solving skills
